What is the 2015 HealthWise Benefit Credit?
This is an annual amount of $130 that Crown contributes toward the cost of your 2015 Health Care coverage. If the program requirements are met prior to December 1, 2014, an amount of $5.00 will be added each pay.
What are the requirements to earn the 2015 HealthWise Benefit Credit (HWC)?
1. Be enrolled in a 2015 Crown Health Plan.
2. Complete the online Health Risk Appraisal (HRA) questionnaire prior to 12/1/2014.
3. Complete a wellness screening prior to 12/1/2014.
Do I have to do both the questionnaire and blood draw to earn the HealthWise Benefit Credit?
YES. BOTH the HRA questionnaire and a wellness screening must be completed prior to 12/1/2014 to earn the 2015 HealthWise Benefit Credit.
What is the online Health Risk Appraisal (HRA)?
The HRA is an online confidential questionnaire about your general health and habits administered by the University of Michigan Health Management Research Center (UMHMRC). The simple questions gather information about general health measure and lifestyle behaviors to determine your individual health status and provide you with valuable information about your health.
What if I donít have access to a computer?
Contact HealthWise immediately. We can help find a computer for you to use. There may be a computer available for employee use at your location. If you live in the New Bremen OH area remember there are computers available for employee use in the Human Resources Building, Education Center, and Celina Security.
What is the Wellness Screening?
The wellness screening consists of a blood draw for a cholesterol profile and fasting blood glucose and is designed to help identify current and future risks for heart disease and diabetes. Of course, the results of the screening are completely confidential.
How do I complete the wellness screening if I am not able to attend a screening at my location?
Crown has partnered with LabCorp to provide this service. To find a LabCorp location in your area, go to www.labcorp.com and click on “Find a Lab” and enter your zip code. If there is no LabCorp or draw site in your area, contact HealthWise for other options. If you live in or near New Bremen OH, you may request a voucher for a draw site in Celina, St. Marys, or Wapakoneta, OH.
I just had a wellness screening (cholesterol profile and fasting blood glucose) done with my doctor. Can I use those results?
If the blood test was completed in 2014 AND you are willing to share your results with HealthWise, that test may be able to meet the wellness screening requirement. Contact HealthWise by email at email@example.com or phone at 419-629-6330 to discuss.
What if I waive coverage under the Crown Health Benefit Plan? Can I still complete the Health Risk Appraisal and Wellness Screening Program?
Absolutely! Crown is interested in the health and well-being of all employees. Of course you would not be eligible for the HealthWise Benefit Credit but you would receive valuable information about your health.
What if I waive coverage but enroll as a spouse/dependent under a Crown Health Plan? Will I earn the HealthWise Benefit Credit in 2015?
If the program requirements are met prior to December 1, 2014, an amount of $5.00 will be added each pay.
My spouse is enrolled in a Crown Health Plan. Can my spouse participate in the Health Risk Appraisal and Wellness Screening Program?

I consider myself a healthy person and I have never been diagnosed with an illness, so why would I want to participate?
Even though you may not have been diagnosed with an illness, it is important to know if you are at risk for a future illness or disease. Many health conditions like certain forms of Heart Disease and Type II Diabetes, for example, can be avoided by being informed and making healthy choices today. Also, early diagnosis and treatment can minimize the effects of serious illnesses and diseases.
How much does it cost?
The HRA and Wellness Screening is a voluntary program and is offered at no cost to you. Crown Equipment Corporation pays the full cost of the HRA and wellness screening.
I am concerned about confidentiality. How do I know that my personal information will not be shared with Crown?
Will my employment status or eligibility for benefits be affected by my health status?
No, health risks or medical conditions have no bearing on your employment status or eligibility for benefits.
